Can you share vdsm logs, to ensure the CloudInit payload (in the CDROM) has been created?

Then please also check CloudInit logs - is it looking for it in the CD?

Just be sure that the cloud-init service enabled before you create the template, otherwise it will fail to initialize a VM.
