We didn’t use the iso at all. If you have vcenter, try something like this (note using vpx, etc):
virt-v2v -ic vpx://username@$vcenter_hostname/$DataCenterName/$ClusterName/$esxiHostName?no_verify=1 $VMName -o rhev -os $EXPORT_DOMAIN --bridge $NetworkNameinOvirt
Here’s our versions, using centos 7:
virt-v2v-1.28.1-1.28.el7.x86_64
qemu-kvm-common-rhev-2.1.2-13.el7.x86_64
qemu-img-rhev-2.1.2-13.el7.x86_64
qemu-kvm-rhev-2.1.2-13.el7.x86_64

The problem seems to be with SASL. Even if I give it a fake vcenter hostname or non-existing virtual machine name, it still gives me the same error with SASL(-7)
