Most likely this failed because of some infra issue (not enough space, some other bad thing re lvm, not sure).
Also adding Sahina. Sahina - perhaps there should be better handling of such failures. AFAIU the failure is in [1]. I guess this specific error message is due to a bug in the 'pv' module. Also we should probably output the error message itself.
