Had same problem on IBM xSeries 3650. Aparently was an RAID problem (Serve RID in my case). I had to wipe the disk, recreate the array an wait until it was OK and only after that I manage to install.

Once it fails, press F2 to drop to shell. /tmp/ovirt.log /var/log/ovirt.log and /var/log/ovirt-node.log will have the debug messages in it.
