One path per controller though its own switch is usually enough.
I install multiple links between storage and switches, to increase the bandwidth of that
link. But I've never done that between switches and hosts.
Link A from host to Switch A to Storage Controller A
Link B from host to Switch B to Storage Controller B

So... I use 4 FC cables (2 per controller) connected from Storage to FC Switch. From this
FC Switch I connect my hosts with 4 FC cables.
The more cables from Storage to Switch, the more throughout you will get.
That way I can use multipath. So when I drop an FC cable or a Storage controller, I
wouldn't lose connection.
In this way it is also possible to use all virtualization technologies, such as LUN
migration between Storage and/or Live Migration.
