Did you perhaps run out of disk space in /tmp? I'm not sure where the virt-v2v does it work, but the error "Conversion of VM from exteral enironment failed: copy-disk stream closed unexpectedly" seems to imply an error there.
I had no issues myself on 4.0.5 (modulo setfiles taking a very long time in a few cases).
