I have seen this problem before. For some reason, oVirt Node 4.1.x does
not always install everything right for efi. In my limited experience,
it fails to do it correctly 4 out of 5 times. The mystery to me is why
it gets it right sometimes. I solve the problem by manually copying the
missing file into my efi boot partition.

I remember I had a bad time trying to disable UEFI on the BIOS of
those servers. In my opinion, the firmware in that model ridden with
problems. In the end, I installed with UEFI (You will need a /boot/efi
partition)
Once installed, I have not had any issues with them.
