How often do these happe
I see one that results in debate about every 2+ years on the one project
I run. You may see them on a release vote during testing if issues are
found or licenses missing etc, typically the a patch is submitted to
resolve it and the vote goes on.
Most time feedback is taken and code is improved through normal working.
